Vaccine Clinic Information

January 18, 2022

Public Health has just announced that they will be hosting a COVID-19 vaccine clinic at Bruce Middle School on the morning of Tuesday, February 8. This clinic is for first and second doses, assuming enough time has passed. I have been informed by Public Health that you would indicate on the consent form that you are giving your informed consent for your child to receive their second dose if they have not reached their 8 week period.

If you would like your child to be immunized during regular school hours (without a parent/caregiver present), submit your consent form via the eConsent Portal or return a completed paper copy of their child’s consent form to the school. If you are unable to access the eConsent your child can bring home a printed version from the school, available in the school office. Both versions of consent need to be submitted by the 24th.

If parents/guardians would like to be present during your child’s immunization you can do so at an afterhours pop up clinic or supersite. In that case, you would not complete the consent form via the e-Consent portal or should not return a paper consent form to the school. Public Health will have consent forms available for families at the pop up clinic.

Please see the attached form from Public Health for more information.
