At times, students may require
medication during school hours. Division policy states that school
personnel cannot administer any medication without the written consent
of parents and authorization of the prescribing physician. If you
require us to administer any medication, please download and complete the form below:

Return the completed form to school with the medication.
Please note that this authorization automatically terminates on June 30th of the each school year or upon change in medication.
