Medication at School

At times, students may require medication during school hours. Division policy states that school personnel cannot administer any medication without the written consent of parents and authorization of the prescribing physician. If you require us to administer any medication, please download and appropriate complete the form below:

Authorization & Administration of Prescribed Medication
(Prescription or Over-the-Counter Medication)

Once you have completed the form, please ensure that your doctor has also signed it. Return the completed form to school with the medication. Please note that this authorization automatically terminates on June 30th of the each school year or upon change in medication.

For over the counter medications such as Tylenol, Advil and Aspirin, only Part I of the form needs to be completed and signed by the parent.
