Dear Parents/Guardians of Grade 6 and 8 Students;

Public Health’s School Immunization Program information for the 2023-2024 year is attached and consists of the Dear Parent Letter (with links to the relevant Manitoba Health Vaccine fact sheets) and the Immunization Consent form.

There will be printed copies of the Immunization Factsheet at your school in the office if necessary.

Consent Form and letter are attached to the email and will also be sent home in a hard copy format.

(Please review the attached documents in this email,) complete the consent form and return it to the school by:  September 15, 2023.

*The Public Health nurses will review your child’s vaccination history and vaccinate only if your child requires it.*
